Linda A Barile Grant Fund
The Dr. Linda A. Barile Grant for HAVEN Participants (“Grant”) was established by HAVEN’s long-time Nursing Professional Director, Linda Barile, PhD, RN. The Grant is intended to support HAVEN participants who require financial assistance to remain compliant with their HAVEN monitoring contract.
Grant Value: $1000
HAVEN may award up to six (6) Grants per fiscal year. Applications are on a rolling basis and are “first come, first serve.” Applicants who do not receive grant monies will have their applications kept on file in the event that more money becomes available.
Participants who are eligible for the Grant fulfill all the requirements listed below:
- Demonstrated financial hardship and making less than $60,000 annually, to be verified by W2.
- Inability to pay certain portions of the financial demands required by the contract, despite good faith efforts.
- Full compliance with all other terms and conditions of the contract, including but not limited to timely submission of reports, attendance at any required meetings, and consistent check ins.
The funds are to be used for HAVEN-related expenses only, as follows :
- HAVEN required Biological Testing
- RecoveryTrek Fees
- SoberLink Monthly Fees
- HAVEN Administrative Fees
All expenses must be well documented and submitted to HAVEN within a week (seven calendar days) of the time they are incurred. The participant is responsible for keeping track of expenses. HAVEN will pay these expenses directly, drawing from the participant’s Grant allowance. Expenses incurred outside or in excess of the Grant amount will not be paid by HAVEN and are the sole responsibility of the participant.
To apply for the grant, please fill out this form completely and submit it to ngreene@haven-ct.org along with the following documents:
- Most recent W2
- Last two monthly bank account statements
- Last two credit card statements
- Documentation of rental income
- Investment income and other benefits
- Documentation of rent/mortgage and other major expenses
Applications submitted without the required support documents will not be considered until all the documents are submitted together.
Participants who submit a completed application with the support documents will have their applications reviewed by a HAVEN committee whose members include the Executive Director, Financial Controller, Administrative Services Director, Board Member, and Development Associate.
